Internal major change request during FR Orientation 2023

This form is intended for advisors to submit major changes for incoming freshmen to Undergraduate Admissions. Please use this form as major change requests come in during in-person conversations during Orientation or via email/phone throughout the summer. 

Remember that incoming freshmen requesting changes into Engineering Science, Dance and Music will require an additional level of review from Admissions or DFPA.  We cannot switch students into Nursing.

Please do not use this form to indicate double majors, minors, etc.

Undergraduate Admissions will process the majors requests in Slate and LOCUS.
